What is the anterior (front) approach for hip replacement surgery?

The anterior approach for a total hip replacement refers to surgery from the front of the hip, rather than from the side (lateral) or back (posterior).  This approach is less invasive compared to traditional hip surgery (posterior).  Using the anterior approach, your surgeon spares the detachment of the hip muscles that help hold the hip joint in place.   By leaving the muscles intact the joint has more stability. This reduces the chance of hip dislocation, one of the risks of traditional hip replacement surgery. 

How does the anterior approach work?

In any total hip replacement, the surgeon smoothes out the hip socket and removes cartilage and any debris, such as damaged bone from osteoarthritis.  No matter the approach (anterior or posterior), both surgeries use similar implants to replace the cartilage and bone.

A cup-shaped implant is secured into the socket. The ball at the top of the leg bone is trimmed away and a metal stem is placed inside the leg bone and secured. A ball (metal or ceramic) is placed at the top of the stem and then a smooth, sturdy plastic, metal or ceramic piece is placed inside the cup-shaped implant. This creates a new hip joint that moves smoothly.

The anterior approach to total hip replacement is becoming more popular because it is less invasive, hospital stays are shorter, and recovery and rehabilitation are quicker. Recovery can be less painful since the incision is smaller and muscles are spared rather than cut.  Most total hip replacement patients whose surgeries were done with an anterior approach are able to bear full weight soon after this surgery, which means they can return to an active lifestyle faster.

Anterior Approach using Mako

The anterior approach combined with Mako technology—the highly advanced, surgeon-controlled robotic arm system—offers an even greater level of surgical precision.  Mako Hip helps the surgeon place the implants in the desired location, providing a good, stable biomechanical reconstruction and acceptable leg length restoration. It is performed with the RIO® Robotic Arm Interactive Orthopedic System. RIO enables surgeons to use a 3-D anatomic reconstruction based on a CT scan of the patient’s own hip to pre-surgically plan implant positioning. During the surgery, it provides real-time data for intra-operative adjustments to further enable surgeons to optimally align and position implants, and accurately reproduce the surgical plan.

Are you a candidate?

Not all hip replacement patients are eligible for this type of surgery. The hip replacement approach will depend on your medical history, a thorough examination, and X-rays or magnetic resonance imagings (MRIs). Oftentimes patients who have implants from a prior hip surgery, are muscular, have a wide pelvis, or are heavier are not well-suited for this procedure.
