Your medical records are maintained by South County Hospital's Health Information Management department and are available for you to inspect and copy.
The privacy and confidentiality of your records is our top priority and, in compliance with the federal Health Insurance Portability and Accountability Act (HIPAA), we provide a Notice of Privacy Practices, explaining how we use and disclose this information, as well as how you can access your medical records.
Request Your Medical Records
Please contact the Health Information Management department for a photocopy or an electronic copy of your South County Hospital medical record or your South County Medical Group medical record (contact information found below).
To expedite the process please complete the Authorization for Use and Disclosure of Protected Health Information form. When filling out the form, please specify what information you are requesting from your medical record and be as specific as possible. Include your/the patient's:

Medical records must be picked up in person. A photo ID is required at the time of pick up to verify your identity.
Office hours are Monday - Friday, 8:30 a.m. - 4:00 p.m. Please check in through the Main Lobby. The medical records office is located on the first floor of the Hospital in the Hazard Wing.
