A Medicare Annual Wellness Visit is a yearly appointment with a Registered Nurse to discuss your personal plan of preventative care in the coming year to prevent disease and improve your health and overall quality of care. One-on-one preventative care!
This is an extra appointment scheduled with a member of our nursing staff that is in addition to your annual physical exam with your clinician.
Check height, weight, blood pressure, and body mass index
Review preventative services such as immunizations and previous diagnostic testing results
Update current list of all healthcare providers
Review relevant health screenings
Provide health education to keep you healthy and safe in the community
Identify needs or barriers to your well-being
Identify community resources that will assist you with your healthcare needs
Provide information and forms on Health Care Durable Power of Attorney and Advanced Directives
After the visit, we will provide a written Personal Preventive Plan to include a schedule of immunizations and screenings
What is not included in a Medicare Annual Wellness Visit?
The Annual Wellness Visit isn’t a routine physical exam that some older adults may get periodically from their physician or other qualified providers. It will not cover illness diagnosis, diagnostic tests (x-rays, ultrasounds, or bloodwork), or treatments for any conditions that you may have. Medicare does not cover routine physical exams.
How much does a Medicare Annual Wellness Visit cost?
There is no cost, deductible, or out-of-pocket fee for any patient that is eligible. This visit is covered by Medicare and Medicare Advantage Plans (Medicare Part C).
How often can I have a Medicare Wellness Visit?
After the first Initial visit, you are eligible for a subsequent Wellness Visit every twelve (12) months.

Starting January 5, masking will be required for all healthcare workers, patients, and visitors in all patient care areas and high-traffic areas (e.g., lobby and waiting rooms) at South County Hospital and all South County Health locations.
