The South County Health Medical Staff Office offers a Hospital Affiliation Verification portal as a service to other hospitals.
Please follow this link to the portal, using the following login and password to enter:
Login: sch
Password: query
Complete all fields, including information about yourself, as well as the first and last name, and last four digits of the provider's social security number.
If you are unable to locate a provider using the query, please contact the Medical Staff Office via e-mail: mso@southcountyhealth.org.
