When you are ready to leave the hospital, your case manager will meet with you and your family to ensure that you have all the information you will need including:
Instructions from the medical staff
Information about any medications you need to take
Dietary restrictions
Signs or symptoms that you should watch for
Activities to avoid
When you can return to work and resume normal activities
Arrangements for any follow-up care you need, such as home health services.
It is important that you understand your discharge instructions and how to follow them, so please ask questions if the instructions are not clear to you. For questions, please call Case Management at (401) 788-1460.
Your Discharge Plan
Please have the family member or friend who will pick you up plan to arrive in the morning, so that the discharge instructions can be reviewed.
Your care team will develop a discharge plan based on your specific medical and health care needs, available services and health insurance.
Typically, patients are discharged before lunch, however, you will not be discharged until your providers complete the necessary medical records.
If your discharge time will be later, your provider or nurse will discuss this with you.
Masking Update
Starting January 5, masking will be required for all healthcare workers, patients, and visitors in all patient care areas and high-traffic areas (e.g., lobby and waiting rooms) at South County Hospital and all South County Health locations. This includes Express Care, Lab, Diagnostic Imaging, Primary Care, Center for Women’s Health, and specialty practices.
