Getting Started

Ask your physician for a referral to the South County Health Diabetes Center and you’ll be on your way to healthier living. On your first visit to the Diabetes Center, we will meet with you individually to assess your diabetes and unique needs. We will give you the tips, tools, and techniques to optimize your health and empower you to make and sustain healthy lifestyle choices. Our goal is to reduce your risk of diabetes complications.

Diabetes Education

To help you manage your diabetes, we give you the option of individual education visits, group classes, or a combination of both. Our programs will help you and your family learn important self-management skills, including:
